The PHI involved names, addresses, dates of birth, social security and drivers’ license numbers, diagnoses/conditions, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE enforced its multi-factor authentication, increased its workforce security awareness training, and increased its threat protections to better protect its electronic PHI.
