OCR description
The business associate (BA), Apex Custom Software, reported that it experienced a ransomware incident that affected the electronic protected health information (ePHI) of 1,500 individuals. The PHI involved included prescription numbers. The BA notified HHS and affected covered entities. In response to the breach, the BA implemented additional administrative, technical, and security safeguards. OCR provided technical assistance to the BA.
