Jacobsen Construction Company Health Plan, the covered entity (CE), reported that it experienced a ransomware attack affecting the electronic protected health information (ePHI) of 2,127 individuals. The ePHI involved included clinical, demographic, and treatment information. The CE notified affected individuals, the media, and HHS. In response to the breach, the CE implemented several corrective actions, including resetting all passwords, requiring users to re-register for multifactor authentication, and providing employees with additional security awareness training.
