OCR description
The covered entity (CE), Magnolia Medical Clinic, PA, reported that an employee mailed the protected health information (PHI) of approximately 1,601 individuals to the incorrect recipients. The PHI involved names of individuals and the name of the clinic. The CE notified HHS, the media, and the affected individuals. In its mitigation efforts, the CE implemented additional quality-control measures to prevent a reoccurrence. OCR provided technical assistance to the CE regarding the HIPAA Rules.
