OCR description
The covered entity (CE), Equality Health, LLC, reported that an unauthorized individual gained access to the electronic protected health information (ePHI) of 9,240 individuals, through a phishing attack. The ePHI involved included clinical, demographic and other treatment information. The CE notified HHS, the affected individuals, and the media.
