The business associate (BA), TAG Urgent Care Support Services reported that it experienced a breach that affected the protected health information (PHI) of 516,985 individuals. The PHI involved included demographic and clinical information. The BA notified HHS, the affected individuals, and the media. In response to the breach, the BA provided complimentary credit monitoring and implemented additional administrative, technical, and security safeguards.
