OCR description
OCR Transaction Number: 26-699203
The covered entity (CE), Midwest Spine & Brain Institute, reported that it was notified by its business associate (BA), 3C Care Systems, LLC, that the BA experienced a breach involving access to its network by an unauthorized threat actor. According to the breach report, the breach affected approximately 108,487 individuals. The protected health information (PHI) involved clinical, demographic, financial and other treatment information. The CE notified HHS, the media, and the affected individuals. OCR provided technical assistance to the CE regarding the HIPAA Rules and obtained documentation substantiating the CE’s notifications.
