OCR description
Anthem Health Plans of Kentucky, the business associate (BA), reported that it experienced a hacking attack that compromised the protected health information (PHI) of 537 individuals. The PHI involved included names, addresses, dates of birth, health insurance information, and claims information. The BA notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect sensitive data.
