OCR description
The covered entity (CE), Memorial Community Health, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 1,658 individuals. The PHI involved included names, dates of birth, and medical billing codes. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE offered free credit monitoring services and implemented additional administrative and technical safeguards.
