OCR description
Urgent Team Holdings, the covered entity (CE), reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 166,601 individuals. The PHI involved included names, birth dates, diagnoses, and medical record numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In its mitigation efforts, the CE strengthened its administrative, technical, and security safeguards to better protect sensitive data.
