OCR description
The covered entity (CE), Family Christian Health Center, reported that it experienced a ransomware attack that affected the protected health information (PHI) of 31,000 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license and social security numbers, health insurance information, diagnoses and conditions, lab results, medications, and other treatment information. The CE notified HHS, the affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
