PIMS Transaction Number: 26-659347 Covered Entity: ABKSW Preferred Health Partners, PLLC, d/b/a North Texas Preferred Health Partners The covered entity (CE), ABKSW Preferred Health Partners, PLLC, d/b/a North Texas Preferred Health Partners, reported that TriZetto Provider Solutions, LLC, its business associate, became aware of suspicious activity within a web portal that some of its healthcare provider customers use to access its systems. The PHI involved included clinical, demographic and treatment information for 2,074 affected individuals. The CE notified HHS, affected individuals, and the media and provided credit monitoring. In response to the breach, the CE confirmed that the BA implemented additional administrative, technical, and security safeguards. OCR also provided the CE with technical assistance.
