Breach Web Description
The covered entity (CE), Open Practice Solutions, LTD reported that it was the subject of an impermissible disclosure of protected health information (PHI) that affected the PHI of 21,265. The type of PHI affected clinical, demographic and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. OCR provided technical assistance to the CE regarding the HIPAA Rules.
