OCR description
The covered entity (CE), Mediclaim, reported that it was a victim of a ransomware attack that affected the protected health information (PHI) of 14,931 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, claims information, and other treatment and identifying information. The CE notified HHS, the affected individuals, the media, and provided substitute notice.
