The covered entity (CE), Lawrenceville Internal Medicine Associates, reported that an employee sent an email containing the protected health information (PHI) of 8,031 individuals. The PHI involved included names and email addresses. The CE notified HHS, affected individuals, and the media. Following the breach, the CE sanctioned the employee responsible for the breach and implemented a secure email messaging system.
