Breach Web Description
The covered entity (CE), Zelis Healthcare, LLC. reported that a mailing error resulted in the impermissible disclosure of protected health information (PHI) of 4,289 individuals. The PHI involved included clinical and demographic information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
