OCR description
The Business Associate (BA), Boutin Jones, reported a hacking incident that affected the protected health information (PHI) of 1,506 individuals. The PHI involved included clinical, demographic, and financial information. The BA notified HHS, the affected individuals, and the Media. In response to the breach, additional technical, and administrative, and security safeguards were implemented.
