OCR description

The covered entity (CE), TennCare, reported that an internal system error resulted in members receiving mail containing the protected health information (PHI) of other members. According to the breach report, the breach affected approximately 2,300 individuals. The PHI involved clinical, demographic and other treatment information. The CE notified HHS, the media, and the affected individuals. In its mitigation efforts, the CE retrained its workforce and implemented additional quality-control safeguards to prevent a reoccurrence. OCR provided technical assistance to the CE.
