OCR description
The covered entity (CE), Northwest Oncology & Hematology, S.C., reported that an employee sent emails to patients without using the blind-copy function. As a result, the email addresses of 1,625 individuals was exposed. The CE notified HHS, affected individuals, and the media. The employee responsible was sanctioned.
