OCR description
The covered entity (CE), At Home Medical, Inc., reported that an inadvertent email error resulted in patients receiving an email containing the protected health information (PHI) of other patients. According to the breach report, the breach affected approximately 1,091 individuals. The PHI involved clinical, demographic, and treatment information. The CE notified HHS, the media, and the affected individuals. In its mitigation efforts, the CE sanctioned the employee involved in the breach and retrained its workforce to prevent a reoccurrence. OCR provided technical assistance to the CE regarding the HIPAA Rules.
