OCR description

The covered entity (CE), MCS, P.C., reported that it was the subject of a phishing attack that affected the protected health information (PHI) of 3,478 individuals. The PHI involved included clinical, demographic and financial information. The CE notified HHS and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained workforce members to better protect its PHI.
