OCR description
The covered entity (CE), L.A. Care Health Plan, reported that it was notified by its business associate (BA), Conduent Business Services, LLC, that the BA experienced a breach involving access to its network by an unauthorized threat actor that affected the protected health information (PHI) of 4,553 individuals. The PHI involved included clinical, demographic, and financial information. The CE notified HHS, the media, and the affected individuals. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
