Superior Care Plus LLC d/b/a Supportive Home Health LLC

PIMS Transaction Number: 26-660991 Covered Entity: Superior Care Plus LLC d/b/a Supportive Home Health LLC Superior Care Plus LLC d/b/a Supportive Home Health LLC, the Covered Entity, reported that on November 20, 2025, it discovered suspicious activity on an employee’s email account. An investigation revealed that a phishing email led to the compromise of the employee’s Outlook Web Access (OWA) account when the employee clicked a malicious link. The PHI involved clinical, demographic and other treatment infomation for 1,208 individuals. In response to the incident, OCR provided technical assistance and the CE implemented additional technical safeguards.
