In compliance with the requirement of the Paperwork Reduction Act of 1995, the Office of the Secretary, Office for Civil Rights, Department of Health and Human Services has submitted revisions to an Information Collection Request to the Office of Management and Budget (OMB) for review and approval. OMB will accept further comments from the public during the review and approval period.
